Appointments
How do I schedule an appointment?
You can schedule an appointment by calling or emailing the ForMotion Clinic location that’s closest or most convenient to you.
What can I expect at my first appointment?
Your first appointment at a ForMotion Clinic will start with an intake interview. We’ll ask you questions about how you’re feeling and what you’d like to accomplish in working with us. This helps to determine your needs and starts a conversation about what type of solution might work best for you. After a brief physical examination, we may make a plaster cast, 3D scan, or take additional measurements, depending on the type of solution you need. We keep a number of solutions in stock at our clinics, but if we need to create a customized solution, we’ll schedule a time for you to try it on and have it properly fitted by a care specialist.
What should I bring with me to an appointment?
If you have a referral form or prescription from your doctor or medical specialist, be sure to bring it to your appointment. If you have other relevant medical documents that might help us better understand your condition, please bring those as well.
How do I cancel or reschedule an appointment?
If you need to cancel or reschedule your appointment, please call your clinic as soon as possible — preferably at least 24 hours in advance. This will help us reschedule you as soon as possible and also make your original appointment available to someone else.

Does ForMotion offer prosthetic devices?
ForMotion Clinic specializes in prosthetic services for upper and lower limbs. Our care specialists are experts who are ready to provide you with the best advice as well as the latest fitting methods and innovations to treat your condition. Our prosthetic solutions are personalized medical devices made to fit and each of our patients comfortably and help them build strength, mobility and dexterity.
Does ForMotion offer orthotic devices?
ForMotion Clinic specializes in pre-made and custom orthotic services for adults and children, ranging from cranial remolding helmets and orthopedic shoes to braces for shoulders, elbows, wrists, knees and ankles. Our care specialists will work with you to understand your condition and goals, and recommend the solution that will work best for you.
Can I get my device repaired at a ForMotion Clinic?
Yes, in many cases, you can. However, we recommend calling first to explain which aid you need repaired and what needs to be fixed or replaced. Over the phone, we can determine whether your aid can still be repaired and/or assess whether it qualifies for replacement or reimbursement. We will arrange for you to visit the nearest location as soon as possible for repair or replacement.
Referrals
Can ForMotion help me with my referral application?
Our clinics cannot make a referral for you, but we can recommend a qualified specialist that you can request a referral for from your primary care doctor. Please note that we can only accept referrals made by a doctor or medical specialist that include a diagnosis as well as the intended purpose of the referral.
Do I need a referral from my doctor to visit your clinic?
To qualify for payment or reimbursement, you will need a referral from a doctor or medical specialist. If you are seeking an orthopedic aid that you plan to pay for yourself, no referral is necessary.
I was referred to a ForMotion clinic, how do I know it was received?
You will receive a written referral from your doctor or medical specialist, which you must bring to your first appointment. Our Clinic staff will confirm this with you when you make your appointment. You can also scan or take a photo of your referral and email it to us.
Payment
What types of payment do you accept?
You can pay with cash, debit card, or credit card at any ForMotion Clinic.

We want every experience with ForMotion Clinic and our care specialists to be as pleasant and beneficial as possible. We’ve built a care culture and professional standards throughout our network of clinics to ensure that all of our patients receive the best care possible.
