What is NDIS?
The National Disability Insurance Scheme (NDIS) provides funding and support for Australians under 65 who have a permanent disability. It’s designed to help people live more independently and achieve personal goals. NDIS funding is tailored to an individual's needs and can include support for prosthetic limbs and related services.

Who is eligible for the NDIS?
You may be eligible if you:
- Are under 65 years of age
- Are an Australian citizen or permanent resident
- Have a permanent disability
- Need long-term support

How do I submit my application?
Fill out the registration forms and submit them online or through your Local Area Coordinator (LAC). Your LAC can assist with the process and answer any questions along the way.The application includes sections for treating providers to complete. Our team can help by filling out the parts that outline your prosthetic assistive technology (AT) needs.
Do I need an occupational therapist (OT)?
If you have an OT, it's very useful to have them complete a section in your application — especially for upper limb clients.Don't have an OT? We can help connect you with one for an assessment.

How Does ForMotion Clinic Work with NDIS Participants?
Steps to work with ForMotion Clinic:
1. Contact the ForMotion Team
- Before your NDIS planning meeting, contact your nearest ForMotion Clinic and ask to speak with one of our Prosthetist. They can help you understand your funding needs and how to prepare for your meeting.
- We will help you define your goals and how prosthetic support can help you live more independently.
2. Create Your NDIS Plan
- During your planning meeting with NDIS, you will work with your Local Area Coordinator (LAC) to develop your personalised plan
3. Use Your NDIS Plan
- Once your plan is approved, book a prosthetic assessment with ForMotion Clinic team
- We’ll provide a functional assessment and submit the necessary quotes in a report directly to NDIS for prosthetic funding approval
4. Prosthesis Approval and Fabrication
- Congratulations!! Now that your prosthesis funding is approved, we’ll begin fabrication.
- Work closely with your prosthetist to ensure everything is in place and be proactive in the process.
